Quality of care and quality of resident services are our top priorities. To keep this goal ever present in our daily lives, we have instituted many systems to ensure fulfillment of our commitment. Our close affiliation with the Geriatric Fellowship and Residency programs at Stanford Medical School is one measure of our commitment. It provides Lytton with a physician on-site at least 3 days per week. This program is under the direction of our Medical Director, who is Stanford’s leading Geriatric Physician. Our Medical Director provides the leadership and sets the standards for patient care. He and his team of licensed physicians, who are seeking board certification in geriatrics, keep us current with the latest and best practices and inspire our staff to do their best. This program alone sets us apart from other facilities in the area.
Quality First Initiative
This initiative is sponsored by AASHA (American Association of Services for the Home and Aging). Lytton is committed to improving the areas that families, physicians, staff and patients see a need. Their comments are valuable and we encourage both positive and negative communication so we may improve our services and programs.
Lytton Gardens institutes a quarterly “Quality Forum” where we review our various “indicators” of quality. This specific system determines action and staff assignments when required. Examples of “indicators” are medication patterns and weight loss or gain trends and review of Resident and Family Satisfaction Surveys. Our commitment to quality improvement provides for and promotes continuous education, reviews procedures and the evolution of processes. Our Safety Committee members are vigilant and are accountable. Members assigned to specific areas of the facility meet monthly to assure the safety of our residents and staff. This ongoing receptiveness to quality of care and service supports Lytton’s core value of “Caring and Respect”.
Lytton Gardens is also participating in the initiative for Advancing Excellence in America's Nursing Homes. Advancing Excellence in America’s Nursing Homes is a two year, coalition-based campaign concerned with how we care for elderly and disabled citizens. This voluntary campaign:
* Monitors key indicators of nursing home care quality
* Promotes excellence in caregiving for nursing home residents
* Acknowledges the critical role nursing home staff have in providing care
