Care Assessment
Admission requests for the Lytton Gardens Health Care Center must be accompanied by the following information:
  • Medical records (history,physical), medical list and progress notes from rehab if appropriate
  • Face sheet and demographics
  • Physician orders
  • Health Insurance ID card – photo copy acceptable
